Job Overview
The Sunnyside Case Manager will provide case management services, intervention, and implementation of client plans to assist in developing, maintaining, increasing, and maximizing independent functioning in the skill areas identified in the Sunnyside Service Plan. Caseloads will consist of families that attend Sunnyside Elementary and surrounding rural communities. Concentration will be on assuring children and families are receiving the proper medical, dental and vision care as well as child development and any additional supportive services.

Responsibilities

  • Work collaboratively with agencies and resources with the community to ensure the families meet service plans objectives
  • Secure and maintain adherence to program consents
  • Ability to maintain confidentially
  • Complete reports of abuse to Child Welfare Services and Adult Protective Services when necessary
  • Must be able to maintain professional boundaries
  • Excellent communication skills, written and verbal
  • Knowledge of community resources
  • Ability to work as part of a team
  • Attend additional trainings as required
  • Make regular home-visits to families
  • Return phone calls promptly
  • Ability to work with people
  • Maintain positive and respectful attitude
  • Ability to clearly communication with people
  • Check emails daily and keep calendar up to date and accurate
  • Working in partnership with school officials
  • Provide parent education
  • Conduct workshops and assist with events
  • Other duties as assigned

Experience

  • Prior experience in social work, behavioral health, or related fields is highly preferred.
  • Demonstrated skills in crisis intervention, crisis management, and addiction counseling.
  • Supervising experience or mentoring background is advantageous.
  • Familiarity with Child Welfare systems, Section 8 housing programs, Medicare, and workforce development initiatives is desirable.
  • Strong communication skills with the ability to engage clients effectively through motivational interviewing techniques.
  • Experience working with diverse populations including children and families is a plus. This role is ideal for professionals committed to making a positive difference through compassionate case management and resource coordination within the community.

Qualifications

  • Bachelor’s Degree in Social Work or two years’ experience in case management
  • Bilingual English/ Spanish is Prefer
  • General computer proficiency
  • Valid Driver’s License
  • Reliable Transportation and Valid Vehicle Insurance
  • Pass Drug Test and background check
  • Availability to work nights and weekends as required
