Job description
This unique position requires the individual to be a parent, grandparent, or sibling of a child born with a special need. The Special Needs Case Manager position is responsible for providing families that have a child diagnosed with a special need case management & linage services.
The family and the SN Case Manager discuss which areas the family is in need of assistance. These goals are set and documented on the Family Individual Service Plan
(FISP). During the course of case management goals are met and updated according to the family needs.
Case Management and navigating resources are offered to all families who have a child with Special Needs. The SN Case Manager is required to provide home and office visits during the course of their case management and emotional support work. It is the CM’s goal to provide each family with the empowerment skills necessary to successfully meet the needs of the family after case management has ended.

SN Case Manager Duties include:
Provide feeling focused parent-to-parent support
Complete necessary documentation for each special needs individual in the client’s care.
Provide Home/Office Visits to each family being case managed.
Transportation Services.
Complete Case Management tools and documentation.
Deliver food/clothes, and other basic needs.
Pre/Post Test 5 Protective Factors and surveys.
Keep cases notes/documentation current.
Provide education and support on IEP/504 plans as applicable.
Educate parents on how to be a successful advocate for their child.
Address client’s needs with referrals and/or Case Management
Type letters/assist with forms and applications for services (SSI) (CCS).
Scheduling client appointments with various social service agencies.
Additional duties as assigned

Minimum Qualifications:

  • BA or 2 years in case management (Preferred)
  • Be the parent, grandparent, sibling or other relative (who has helped raise the sibling) of a child diagnosed with a special need
  • Must have reliable transportation
  • Valid CA Driver’s License /Ability to pass a Criminal Background Check

Job Type: Full-time

Salary: Starting Wage: $19.00

  • 7.5 hour shifts / Day – (typically M-F)
