Job Title: Special Needs Services Case Manager

Wage: $17-18/Hourly
Job description
This unique position requires the individual to be a parent, grandparent, or sibling of a child born with a special need. The FRC/Special Needs case manager position is responsible for providing families that have a child diagnosed with a special need case management services. The family and the FRC/SN case manager discuss which areas the family is in need of assistance. These goals are written down on the Family Individual Service Plan (FISP). During the course of case management goals are met and updated according to the family needs.
Case management is offered to all families who have a child 0-5 years of age. The case manager is required to provide home visits during the course of case management. It is the CM’s goal to provide each family with the skills necessary to successfully meet the needs of the family after case management has ended.
Case manager duties include:
Provide feeling focused parent-to-parent support
Complete First 5 information packet on each child 0-5 in the client’s home
Provide Home Visits to each family being case managed
Transportation
Complete Case Management tools (FISP)
Deliver food/clothes
Pre/Post Test 5 Protective Factor Survey
Keep cases notes current
Provide education on the IEP/504 plan
Educate parents on how to be a successful advocate for their child
Address client’s needs with referrals and/or Case Management
Type letters/assist with forms (SSI) (CCS)
Scheduling client appointments with various social service agencies
Additional duties as assigned
Minimum Qualifications:
BA or 2 years case management preferred
Be the parent, grandparent, or sibling (who has helped raise the sibling) of a child diagnosed with a special need
Must have a vehicle
Valid CA Driver’s License /Ability to pass a Criminal Background Check
