Every year, DDS sends a Family Outcomes Survey to a randomized group of thousands of families who have participated and are participating in the Early Start Program. We are excited to share that DDS has sent the annual survey to families.
What you need to know…
This survey is meant for DDS to use your feedback to create the best experience possible for families participating in the Early Start program. We encourage you to complete the survey on or before October 31, 2023. While the survey asks for some information about your family, including their child’s birthdate, please rest reassured that the information is coded in an anonymous format and is only used in aggregate data.
